Scenarios/case studies for root cause analysis

We are looking to develop a root cause analysis process for incident investigation and review. We are currently in the process of documenting some scenarios/case studies that can be used in our root cause anlysis training but would be interested to receive and share any scenarios/case studies other organisations might be willing to share.

There are some case studies on the website developed during the old DSOP project in relation to risk assessments under manual handling, working at external locations and working alone under the hazards/risk tabs which you could adapt for some root cause analysis if you need them. I also have some old scenarios in hard copy around cllients refusing to replace old and defective equipment and client behaviours if you would like me to fax them to you.
